“It could be that people who spent less time outdoors did so because of adhering to the stay-at-home orders and felt deprived of their ability to be outdoors. Or that people who reported spending more time outdoors did so because they were experiencing poor mental well-being,” she said.

Dr. Mandelabum, however, said that this should not detract from the main finding of the study.

“I would focus more on the fact that [more people] improved by going outside, being in nature, and exercising in a high intensity way, which optimizes anxious and depressed states. [T]he real conclusion is the importance of physical activity and being outdoors,” he said.

Dr. Young noted that other findings were mostly consistent with earlier research.

“Many previous studies have documented that females tend to have higher anxiety and depression scores than males. There are also a number of studies suggesting that older adults have better mental well-being than younger adults,” she told MNT.

The research attributed the latter to adults having lived through more adversity and having more experience in dealing with stress.

According to Dr. Merrill, the new study highlights an important issue. He said that his colleagues have seen a disproportionate number of adolescent patients facing challenges with their mental health since the pandemic.

“The data is confirmation of the experience of mental health clinicians working in these populations — that younger adults, in particular, have struggled during the pandemic with disruption of their normal activities, [having been ordered to] stay at home, restricted from being in outdoor public spaces. This really comes out in the data that these restrictions had a significant negative effect on people’s well-being,” he told MNT.

“Living through that disappointment of not being able to do something as normal as going to the park can be a very big setback for a young person who otherwise may have been just having a nontraumatic, normal upbringing and a normal life.”

Dr. Young expressed some surprise at the racial differences in depression and anxiety scores.

“Asian Americans tend to report lower depression and anxiety compared with white people. On the other hand, Black people tend to have higher scores than white people, so this was unexpected, and I don’t believe it’s reflective of the general population,” she said.

She said this finding could be down to only 2.3{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} of the cohort being Black adults.

“As far as the difference in the levels of depression and anxiety in terms of race and ethnicity, the data speaks for itself. There’s no definite explanation,” said Dr. Merrill.

For Dr. Mandelbaum, the tools that the team used to measure anxiety and depression levels might explain the finding.

“The tools, these objective measures, and the data pointed out cultural, racial differences. Looking at those in a nonbinary way can be misleading. I think the tools are limited because if you have 10 people in a room, and there are 10 people from 10 different countries, even if you’re asked the question in the same language, you will see how different people interpret it. These tools are very hard [to optimize] across different cultures or languages,” he said.

Meanwhile, Dr. Merrill said that the finding could possibly reflect an often overlooked reality for racial minorities.

“If you’re already more under stress based on systemic exclusion, or marginalization, [the pandemic may not have had] such a big impact. For a population who is already under stress, if you add on more stress, there may be less of a change compared with a group that has enjoyed privilege or a stress-free life. In the U.S., that kind of seems to be the reality,” he explained.

The study shows that future pandemics will need better planning and more careful thinking around people’s mental health. This could affect decisions to close down outdoor spaces, for instance.

Dr. Young acknowledged that fighting a novel virus has been tricky for all countries.

“I’m sure we all would like to have consistent messaging throughout the pandemic, but as the knowledge changes, so do the recommendations,” she said.

However, the study’s findings reiterate that even during an active pandemic, physical activity or spending time outdoors could help people maintain their physical and mental health.

“Governments should promote physical activity and spending time in nature during pandemics. Public health messages have been consistent about getting vaccinated, staying masked-up, frequent hand washing, and maintaining physical distancing. It would be great if they can include positive messages like going for brisk walks.”

The researchers used an exercise test called the 6-minute walk test to assess the persistent effects of SARS-CoV-2 infection on cardiovascular function in women during the post-acute phase of the infection.

They found that women with COVID-19 who had mild-to-moderate illness during the acute phase showed a slower decline in their heart rate after the 6-minute walk test than the participants in the control group. This difference was more pronounced in women actively experiencing long COVID symptoms.

However, encouragingly, they found no statistically significant differences in a range of other measures, including pre- and post-test oxygen saturations, pre- and post-test heart rate, ratings of perceived exertion, and ratings of perceived breathlessness.

The study included 29 women who had experienced mild or moderate illness due to COVID-19. These women had received a positive SARS-CoV-2 diagnosis at least 4 weeks prior to the study, with the average time of diagnosis being 94 days before the study. The control group consisted of 16 women who had never tested positive for a SARS-CoV-2 infection.

Out of the 29 participants in the test group, 17 women reported experiencing at least one long COVID symptom at the time of the study. These long COVID symptoms included shortness of breath, fatigue, cough, muscle or joint pain, loss of smell or taste, and skin irritation.

The researchers administered pulmonary function tests to assess various aspects of lung function. They found that the women who had tested positive for SARS-CoV-2 exhibited reduced total lung capacity compared with those in the control group.

The researchers then evaluated changes in cardiovascular function during and after the 6-minute walk test. The test measures the distance that participants walk in 6 minutes at their normal pace, and researchers use it to assess exercise tolerance. The team adjusted the results for age, sex, and body mass index (BMI) to prevent these variables from influencing the test results.

There was no significant difference between the distances that the two groups covered during the test.

However, women with SARS-CoV-2 who experienced shortness of breath or joint or muscle pain had a lower adjusted distance value than those with SARS-CoV-2 who did not experience these long COVID symptoms.

The researchers assessed the blood pressure, heart rate, and blood oxygen or oxygen saturation levels before and immediately after the 6-minute walk test. The heart rate response during exercise is a measure of cardiovascular function, with a lower heart rate response being a predictor of reduced exercise capacity and cardiovascular disease-associated mortality.

The women in the SARS-CoV-2 group showed a lower heart rate response than those in the control group.

Moreover, the participants in the SARS-CoV-2 group who reported shortness of breath as a long COVID symptom at the time of the study had a lower heart rate response during the test than those in the SARS-CoV-2 group who did not have this symptom.

The Pfizer-BioNTech and Moderna COVID-19 vaccines are both nucleic acid-based medicines. Nucleic acids in these vaccines are enclosed and protected by lipid nanoparticles that deliver them across the host cell membrane.

Although they are effective, drugs delivered in this way are vulnerable to degradation during storage.

One delivery method for nucleic acid-based medicines is electroporation. This involves delivering nucleic acids to cells made more permeable using a pulse of electricity.

While somewhat effective, these electrical impulses can cause muscle contractions, tissue damage, and pain, and they may not be suitable for people who use implantable electrical devices such as defibrillators or pacemakers.

Using this method also requires substantial training, and equipment costs may make it inaccessible to regions and populations with fewer resources.

Creating a method for delivering nucleic acid-based medicines that overcomes unwanted side effects and cost barriers while still being effective could increase the accessibility of vaccines to areas with limited resources.

Recently, researchers from Rutgers, the State University of New Jersey in Piscataway, and GeneOne Life Science in Seoul, South Korea, developed a method based on an ancient cupping technique to deliver nucleic acid-based medicines.

“Development of enhanced delivery technologies plays an instrumental role in bringing nucleic acid-based biologics to broad use and clinical relevance, and worldwide vaccine distribution is just one example,” said senior study author Prof. Hao Lin.

Cupping therapy is an ancient treatment method from the Middle East and China. It involves applying heated cups to the skin to create negative pressure and increase blood circulation to promote healing.

Since 1950, doctors have used the method in hospitals in China and elsewhere for conditions such as acne, facial paralysis, and dyspnea.

Although evidence of its health benefits is lacking, the authors of the new study decided to investigate a similar technique as a potential delivery method for nucleic acid-based medicine.

To test the method, the researchers gathered two groups of rats and injected both with DNA in the upper layers of their skin. One group received the “suction cup treatment” on their injection site, whereas the other group did not.

The researchers monitored the activity of the DNA with fluorescence microscopy. They found that gene expression from the vaccine was detectable at 4 hours post-vaccination. In the rats who also underwent cupping, however, gene expression was detectable at 1 hour post-vaccination.

At 24 hours post-injection, the researchers saw that gene expression was detectable eight times deeper under the skin among the rats who underwent cupping than among those who received DNA injections alone.

To investigate whether or not this technology could work to deliver potential COVID-19 vaccines, the researchers next injected the rats with a synthetic SARS-CoV-2 DNA candidate vaccine.

They split the rats into three groups:

  • The first group received two injections of the vaccine candidate without suction on days 0 and 14.
  • The second group received one vaccine, followed by dermal suction on day 0.
  • The third group received two vaccinations and dermal suction on days 0 and 14.

The researchers collected blood samples from the rats on days 0 and 14, before vaccination, and again on day 29.

They found that the rats who received cupping after the vaccine had a significantly stronger immune response than those who received the vaccine alone.

The researchers also found that immune responses in the rats who received a single vaccine followed by suction were not statistically different from those who received two vaccines followed by suction.

They also note that the rats who underwent suction had no evidence of tissue damage or lymphocyte infiltration in their injected areas.

The researchers say that the underlying mechanisms for suction-enhanced delivery are not clear. They hypothesize, however, that it may work via the CG pathway. This plays a role in determining which molecules travel through membranes and enter cells.

The researchers explain that it will be challenging to confirm the underlying mechanisms of cupping due to the complexity of the processes involved and a lack of availability of drugs used for testing.

Nevertheless, the researchers say that even without mechanistic understanding, it is still possible to improve the method.

“Currently, the procedure is two-step: an injection followed by suction,” Dr. Lin told Medical News Today.

“We are working on a combination device/method to render one-step application — that is, injection and suction in a single step by a single device,” he continued. “Furthermore, we are trying to reduce suction time even further to increase patient and staff acceptance.”

When asked whether this method could work for COVID-19 vaccines, Dr. Lin said, “Theoretically, this may be possible, but we cannot answer without testing since, except for DNA vaccines, COVID-19 vaccines are delivered into the muscle, whereas this device delivers [the] vaccine to the upper layers of the skin.”

“The [suction] device is being tested currently in phase 1 and phase 2 clinical trials of a COVID-19 vaccine. The documentation to enable filing with regulatory agencies for general and widespread clinical use is being readied,” he said.

The researchers conclude that cupping is a promising method for nucleic acid-based medicine delivery. However, further research is necessary to see whether it could work to deliver COVID-19 vaccines to humans.

“The suction device has numerous advantages relative to other devices for DNA vaccine delivery. First, it is very inexpensive and easy to use, requiring minimal training,” Dr. Lin told MNT. “Secondly, the device is run from a battery that is easily rechargeable.”

“Third,” he continued, “is the high degree of patient acceptance, as the device will help deliver DNA vaccines without pain or discomfort.”

“And fourth, unlike the significant setup times with other devices, this only requires changing out a cap and then pressing the power button; the machine will turn off in a few seconds.”

“This, coupled with the many advantages of DNA as a vaccine type — the greatest advantages being the ability to completely avoid the need for freezers from the clinician’s perspective, and low side effects and discomfort from the patient’s perspective — makes this an ideal vaccine platform for resource-limited and remote regions,” he concluded.

Mandy Cohen, secretary of the state Department of Health and Human Services, has talked about her daughters many times as she helped steer the state through the COVID-19 pandemic.

Now the state gets to see them in a public service announcement about the kid-sized Pfizer COVID-19 vaccine that has just become available for children ages 5 to 11.

Cohen, the proud mother of a 7- and 9-year-old, took her children on Saturday to get a first dose of the two-dose Pfizer vaccine approved last week for emergency use by the Food and Drug Administration and the Centers for Disease Control and Prevention.

“Like most kids, my daughters don’t like shots,” Cohen told reporters at a briefing on Wednesday. “But we talked as a family about the reasons it was important to protect them from COVID just like we protect them from flu and other childhood illnesses like chicken pox.”

Then she showed a video that she said she hopes will help persuade other parents of the safety and efficacy of the vaccine. In clinical trials, according to data reviewed by Cohen, the CDC and FDA, the kid-sized dose of the Pfizer vaccine, a third of the amount in the vaccines given to anyone 12 and older, is 90 percent effective at preventing COVID infection in the younger children.

“We love our kids so much and we want to make good decisions for them and their health, and I was really happy that we have a safe and effective COVID vaccine,” Cohen says in a video showing her hugging her daughters. “And I was really proud of them today that they got their vaccine. It was a huge sigh of relief for mom. So I hope everyone goes out and gets their COVID vaccine for their kids as soon as possible.”

Cohen posted photos of her daughters to her Twitter account on Saturday. There is one of her older daughter getting her vaccine. Another one shows the two girls together after getting their shots. The 9-year-old gave a thumbs-up while revealing the colorful Band-Aid covering the spot on her left arm where she got the shot. The younger girl is smiling with her hands raised in the air.

They might have been celebrating, too, because chocolate milk was coming to them after their shots, their mother said.

Cohen’s girls are among the more than 24,000 children in North Carolina who have received their first dose of the newly available vaccine. Though children can experience side effects such as a sore arm, fever, headaches or feeling tired for a few days, Cohen said her daughters experienced no side effects.

“We have plenty of vaccine supply across the state. Kids can get vaccinated at any location that has a smaller dose of the Pfizer vaccine available,” Cohen said. “This includes their pediatrician or doctor’s office or hospital. Unlike other vaccines, younger children can also get vaccinated at local pharmacies and grocery stores, making it even easier for parents to find a convenient location for them.”

Additionally, DHHS has worked with community partners to set up nine family COVID-19 vaccination centers in historically underserved communities that will be open for the next six weeks. The centers will be open on weekends and during the evening so parents don’t have to take their children out of school or miss work to get a vaccine.

The centers also will have vaccines available for people older than 12, as well as boosters. Spanish interpreters will be there, too.

Cohen and Charlene Wong, a Duke pediatrician and the DHHS assistant secretary for children and families, know that many parents might have questions about the vaccine and tried to provide answers to many at the briefing with reporters. They also encouraged parents to talk with their pediatricians and other trusted health care providers about any concerns they have.

Here are some of the questions and answers:

Q: Two physicians asserted in a recent Wall Street Journal opinion piece that a low number of children in this country have been hospitalized or died from COVID infections. But the piece also cited data that have not been peer reviewed and have been contested.

So, what’s the urgency to get the younger children vaccinated?

A: To date, North Carolina has seen more than 1,300 hospitalizations and 11 deaths in children under the age of 17, out of 244,902 cases, according to data on the DHHS COVID-19 dashboard. Cohen and Wong made the point that although the risks of hospitalization and death from COVID are rare in children in that age group, the virus can make them very sick.

Some children infected with COVID have come down with the multi-system inflammatory syndrome, or Mis-C, a rare condition in which their organs can become inflamed and in some cases can be deadly.

Children also can experience long-COVID symptoms such as fatigue, headaches, insomnia, trouble concentrating, muscle and joint pain, as well as a lingering cough, months after infection, according to the CDC

“We are seeing that children can have longer-term side effects, fatigue, headache, muscle aches, lethargy, again, different kinds of tiredness. So we are seeing that in children and adults,” Cohen said. “And so I think what we want to do is say we have a tool here that is safe and effective at preventing your child from getting COVID in the first place. Let’s use that safe and effective tool to make sure they don’t get COVID.”

Q: How prevalent is long-COVID in children?
