They estimated glomerular filtration rate (eGFR) at baseline and follow-up visits and defined incident CKD as eGFR that decreased to below 60ml/min/1.73 m2. eGFR measures how well the kidneys are removing waste from the body. The healthy range is 90-120 ml/min/1.73 m2.

The 25,570 participants were ages 49–77 years at the beginning of the study. Overall, 4,944 (19.3{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809}) developed incident CKD during the follow-up period of, on average, 11 years.

People who had higher levels of seafood-derived omega-3 in their diet had an 8{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} lower risk of incident CKD. Although the findings from the study are promising, the results do not indicate a causal relationship.

“It should be noted that our findings do not prove a causal relationship between seafood n-3 PUFA and CKD risk as the findings were observational, but they are supportive and consistent with current clinical guidelines that recommend adequate intake of seafood as part of healthy dietary patterns, especially when seafood replaces the intake of less healthy foods,” Dr. Ong said.

Dr. Laird, meanwhile, called the findings “controversial,” and said many other compounds could have had a similar effect.

“[T]he authors did not take into account other dietary factors (including other vitamins (e.g., vitamin D) and minerals found in fish) which also have anti-inflammatory benefits which the EPA and DHA levels are just tracking against,” he noted.

Under the new approach, most people would be advised to simply get whatever the latest version of the vaccine is annually each fall like the flu vaccine. They wouldn’t have to worry about how many shots they’ve already gotten and which one they got when. Those who still need to receive two doses initially, such as young children and older people, would use the same formulation for all three shots.

Vaccine makers would update the annual shot through a process that would begin each spring to try a match the vaccine as closely as possible to whatever variant will likely be dominant in the coming winter. That’s how the flu vaccine is formulated each year.

“FDA anticipates conducting an assessment of SARS-CoV-2 strains at least annually and to engage VRBPAC in about early June of each year regarding strain selection for the fall season,” the FDA wrote in its briefing document, adding that updated vaccines would be readied for use by September each year.

The agency notes that if a more dangerous COVID variant were to emerge, it might reconsider the vaccine strain at other times of the year on an “as-needed and emergent basis”

Some immunologists and vaccine researchers say simplifying the process along the lines of the flu vaccine is appropriate at this point in the pandemic. However, many questions remain about emerging booster strategy.

“As far as the tools that we have right now, I think it just makes the most sense to plan to update each year as close as we can to the currently circulating variant,” says Deepta Bhattacharya, an immunologist at the University of Arizona. “So I think all the things the FDA is considering make a lot of sense.”

Questions about efficacy of updated shots

There’s an intense debate about the wisdom of updating the COVID vaccines regularly to try to match new variants. Some researchers question whether that really makes the vaccines more effective. They also argue the low demand for the latest booster shows the public has little appetite for continued boosting with the vaccines, even if they’ve been updated with new strains.

“The public is voting with their arms if you will and said, ‘No. I’m not going to get this. This doesn’t make sense to us,'” says Dr. Gregory Poland, a vaccine researcher at the Mayo Clinic.

While endorsing continued boosters for those at high risk, such as the elderly, some question whether the current bivalent vaccines updated to target omicron have enhanced protection compared to the original vaccines. Most people are still well-protected against severe disease by the immunity they already have, they say.

“We have no solid data about the performance of the bivalent boosters,” says John Moore, an immunologist at Weill Cornell Medical College. “The hard evidence is lacking, and the evidence that is out there is at the very least inconclusive and to me trends towards saying the bivalent boosters were little if no better.”

Moore and others argue the virus is changing so fast that it’s pointless to constantly try to match the vaccines to the latest variants.

“We shouldn’t really be chasing these variants, which are evanescent and are often gone by the time you’ve created the vaccine,” says Dr. Paul Offit of the University of Pennsylvania, one of the FDA’s advisers.

Offit and others also question whether everyone will necessarily need to be boosted regularly, or just those at high risk, like the elderly.

Arguments for alternate vaccine strategies

Critics of the FDA’s proposed new strategy argue it would be better to invest in developing better vaccines that might be more appealing to people, and in campaigns to get more people vaccinated. Better vaccines could include those that could keep people from catching the virus in the first place not just from getting seriously ill — such as a nasal spray vaccine. Or perhaps vaccines that provide longer protection or are administered in pill form, to make them more acceptable to the needle-averse.

“Particularly now when Congress is not allocating new funds for COVID response, we have to be especially judicious in how we spend our money and what would be most cost-effective,” says Dr. Celine Gounder, a senior fellow at the Kaiser Family Foundation. “It’s unclear whether updating the booster formulations and repeatedly boosting people is the most effective approach to controlling COVID at this stage.”

Another concern some researchers have is that the FDA continues to rely on antibody levels to test vaccine efficacy.

“I think we need to raise the bar and require more evidence of clinical efficacy,” says Dr. Eric Rubin, a professor of immunology and infectious diseases at Harvard who is also a member of the advisory committee. For instance, Rubin says the FDA should require proof the updated vaccines are actually reducing the risk of getting infected, getting sick, hospitalized and dying.

Others say updating the vaccines make sense to make sure people are as well-protected as possible while researchers continue to try to develop new vaccines.

“Even if you don’t have a booster that matches 100{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} what’s circulating, you will have a booster that matches 75{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} to 80{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} to 90{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} of what’s circulating,” says David Martinez, an immunologist at the University of North Carolina, Chapel Hill. “And that will be good enough. I would probably benefit most people.”

FDA looks ahead to endemic COVID

Some researchers think it’s too soon to rely on annual boosters. COVID hasn’t quite settled into a seasonal pattern like the flu, they note, and the SARS-CoV2 virus is changing more quickly than the flu virus. So people may need to be boosted more frequently, especially since protection against severe disease may only last about four to six months, they say.

“We’re going to be reaching that pretty soon with the early adopters of the bivalent boosters, like myself,” says Dr. Peter Hotez, a vaccine researcher at the Baylor College of Medicine. “I don’t know if an annual strategy is going to cut it.”

The FDA’s plan comes as COVID appears to be moving towards becoming an endemic disease. That doesn’t mean it’s going away or it’s not a threat anymore. The disease will continue to be a threat for the foreseeable future, making many people sick and even potentially killing hundreds a day, making it a major public health problem and a leading cause of death, health experts say.

But unless some more dangerous version of the virus suddenly emerges, the world might finally be settling into a more predictable co-existence with the virus. The federal official NPR spoke to says the goal of the new vaccine strategy is to make the vaccines, which are the major weapon for protecting ourselves, simpler and hopefully therefore more appealing. The latest boosters have found very few takers.

The thinking is that at this point in the pandemic the overwhelming majority of people have a significant levels of immunity, either from having gotten vaccinated and boosted, or infected one of more times, or both. And while that immunity appears to protect most people from severe disease, that protection does appear to fade with time.

The FDA is also considering making the shots interchangeable. That way people wouldn’t have to worry which brand they’re getting. Again, the change is aimed at making COVID shots more like the flu shots. People don’t typically worry about the brand of the flu vaccine they receive.

The vaccine would still be administered at different doses for different ages. And very young children and older people would still get two shots each year, much like the flu vaccine.

If the FDA advisory committee endorses the approach Thursday, the FDA would work with the vaccine companies and Centers for Disease Control and Prevention to finalize the details. And the FDA advisers would meet again in the spring to pick the specific strain or strains of the virus the new shots should target.

The study was conducted by researchers from the Finnish Institute for Health and Welfare, the University of Helsinki, and the University of Eastern Finland.

Previous studies, such as one published in 2022, concluded that individuals with depression levels that were increasingly severe, chronically high, or chronically low had a higher likelihood of developing dementia compared to individuals without depression or with decreasing depression symptoms.

Other studies have found that anxiety, vital exhaustion, and psychological stress are associated with a later onset of dementia.

On the other hand, a 28-year follow-up study on the relationship between depression and dementia found that depression was mostly caused by dementia rather than dementia being caused by depression.

Since psychological distress is common in the early stages of dementia, risk-determining studies must have a sufficiently long gap between the measurement of psychological distress and the incidence of dementia for the result to be considered reliable.

Studies with an elderly population and short follow-up periods fail to separate the early (prodromal) symptoms of dementia from causal risk factors.

Another factor to keep in mind in studies on psychological distress and dementia is the competing risk of death. According to some authors, studies should take into account whether people with mental health problems tend to die at younger ages and thus may not live long enough to show dementia.

“We could clarify that connection using one of the largest population data sets, long follow-up, and careful modeling of death [from] other causes,” Dr. Sonja Sulkava, principal investigator for the study and postdoctoral researcher in Professor Tiina Paunio’s group, told Medical News Today.

The study included 67,688 individuals, ages 25–74, who participated in the National FINRISK Study surveys between 1972 and 2007.

FINRISK was a large Finnish population study on risk factors in chronic, noncommunicable diseases that was carried out for 40 years, and its surveys included questions on symptoms of psychological distress.

Dementia and mortality data for each participant until December 31, 2017, was obtained from the Finnish Health Register.

Taking into account the competing risk of death and other factors that affect the risk of dementia, the researchers found that symptoms of psychological distress were associated with a 17-24{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} increased risk of dementia in an etiological Poisson model and with an 8-12{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} increase in the incidence of dementia in the Fine-Gray model.

Dr. Moffitt expressed confidence in the findings of this study, noting that the results converge with those of a New Zealand population study.

“In 2022, my team also reported that mental health is an early factor in later dementia. We followed 1.7 million New Zealanders for 30 years in national medical records and found that early-life mental disorder predicted 4 times greater risk of later-life dementia,” he said,

Like the Finland population study, the New Zealand study also controlled for the competing risk of death.

Dr. Linda Ernstsen, associate professor at the Norwegian University of Science and Technology, who was not involved in the study, told MNT:

“The take-home message from this study is that mental health issues and feeling of distress are associated with premature death and dementia. These findings illustrate the need to focus on mental health at all ages and to identify causes and triggers.”

In their paper, the researchers note that individuals who did not participate in the FINRISK survey or had missing information also had more risk factors and increased risk of dementia and mortality, and this selective participation and non-responsiveness may skew the study results.

The researchers also acknowledged that their measurement of psychological distress was not based on a validated multi-item questionnaire but on several single-item measures for different symptoms of psychological distress. However, these measures correlate significantly and show a consistent pattern of association with dementia.

Moreover, the researchers acknowledged that there was no available information about traumatic brain injury, hearing impairment, and low social contact—three established risk factors for dementia.

Dr. Sulkava added that study participants were asked to report their current symptoms of psychological distress only once, and this leads to a lack of “longitudinal perspective” for the symptoms.

Dr. Ernstsen pointed out that information about social isolation or marital status was not included. Research has found that being married protects against dementia.

“We also know that cardiovascular disease is associated with both mental health and dementia risk, but it was only the presence of diabetes that was adjusted for in the present study,” Dr. Ernstsen added.

Dr. Archana Singh-Manoux, research professor and director at the French Institute of Health and Medical Research (INSERM), who was not involved in the study, told MNT that the major limitation of this study relates to reverse causation.

In one of the models used to calculate dementia risk, when the researchers excluded individuals with a follow-up of fewer than 10 years, the sensitivity analyses showed no significant associations between psychological distress and dementia.

This is a perfect demonstration of reverse causation, i.e., psychological distress in the preclinical phase of dementia rather than psychological distress ‘causing’ dementia.”

When asked about the next research steps, Dr. Sulkava told MNT that larger studies and longer follow-ups will be needed.

“Stress, exhaustion, and depressive symptoms are tightly linked to sleep problems, which are suggested to be risk factors for dementia too. However, most epidemiological studies lack large enough sample[s] or long follow-up,” she said.

“Our next step is to study sleep problems and sleep length and dementia risk using the large Finnish cohorts,” she added.

As many as 40{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} of Europeans have a vitamin D deficiency, with people with dark skin more likely to be affected. Another group more likely to be affected by vitamin D deficiency are people with obesity.

The scale and type of impact of this are unknown, but vitamin D deficiency has been blamed for rising rates of type 1 and type 2 diabetes, multiple sclerosis, and various autoimmune conditions.

This deficiency has also been linked to cancer risk and poorer cancer treatment outcomes. Obesity is also a risk factor for many of these conditions.

Increasingly, research has shown that supplementation with vitamin D in people with obesity, defined as having a BMI over 30, does not correct the deficiency as effectively as it does in people with a BMI under 25.

Dr. Linia Patel, consultant dietitian, sports nutritionist, and British Dietetic Association spokesperson, told Medical News Today in an interview that her own research on a cohort of 2,842 individuals under 40 in the United Kingdom showed that vitamin D deficiency was associated with higher BMI and waist circumference.

BMI is a measurement that infers overall adiposity, or body fat, and the waist measurement infers the amount of visceral fat an individual may have in their abdomen.

“We’ve got two issues going on,” said Dr. Patel. “We have seen that […] people […] who are obese have lower vitamin D, starting off. And then even when you supplement them, their levels of vitamin D don’t go as high as people having ‘normal weight.’ So that’s one issue.”

“If you are having a higher BMI, you are more at risk of cardiometabolic risk factors, and what’s fantastic for me, as a public health dietitian, is [that] if we supplement people with vitamin D, we reduce that risk,” she told us.

The issue was that vitamin D supplementation advice is “one size fits all,” with little consideration given to different populations, she said.

The potential role vitamin D deficiency can play in increasing the risk of developing different conditions is controversial, with many trials and studies reporting inconclusive results.

It is also possible that the trials were asking the wrong questions in the first place, said Dr. Patel.

“A lot of other research that has been done into vitamin D hasn’t looked specifically at the right question. So you can see that the research is conflicting in this area. And a lot of research actually shows that vitamin D supplementation doesn’t make a difference, but some shows then that vitamin D supplementation does make a difference in reducing your risk factors.”

“And I think the papers that showed that it doesn’t make a difference, are perhaps not using a high enough dose, and perhaps not asking the right questions in terms of what, in fact, are we looking at? Are we looking at when people die? Are we looking at their risk of diseases? So I think we just need more specific and good quality research in this area,” she added.

The VITAL study sought to determine the effects of vitamin D and omega-3 supplementation on cancer and cardiovascular events in a cohort of 25,871 adults in the United States.

The initial results from the trial were published in the New England Journal of Medicine in 2019 and showed that while vitamin D supplementation did not reduce the risk of cancer, cardiovascular disease, or bone fractures in the cohort, the risk of cancer-related death was reduced with supplementation.

However, further analysis of the results told a different story when stratified by BMI. For people who had a BMI under 25, vitamin D supplementation was associated with a 24{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} lower cancer incidence, 42{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} lower cancer mortality, and 22{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} lower incidence of autoimmune disease compared to placebo.

This effect did not occur in people with overweight or obesity, which led the researchers to carry out a further analysis to quantify the difference in the impact of vitamin D supplementation on people with different BMIs. The results of this research appear in JAMA.

Researchers looked at the vitamin D levels of 16,515 participants before and after supplementation, as well as their BMI.

This paper shows that those with a BMI below 25 were less likely to have a vitamin D deficiency in the first place, and more likely to see their vitamin D deficiency corrected with vitamin D supplementation than those with a BMI over 30.

The authors proposed that this difference could be due to the difference in the activity of the parathyroid hormone, which can stimulate vitamin D production in the kidneys when a person has low levels, in people with a higher BMI.

In people with a BMI under 25 researchers expect that there is an inverse relationship between vitamin D levels and parathyroid hormone levels, but the level at which vitamin D supplementation can lower parathyroid hormone levels is likely to be higher in people in obesity than in people without.

Prof. JoAnn Mason, professor of medicine at Harvard Medical School, and principal investigator of the VITAL trial told MNT that “[t]he parathyroid hormone didn’t come down, […] it didn’t reduce as much in response to supplementation in those with higher BMI.”

Next, the team wants to investigate whether or not the dosage of vitamin D makes any difference to people at different BMIs as well as looking at the gene expression that occurs in people when they are given vitamin D supplementation.

Prof. Mason added: “We really believe that more attention [should] be given to body mass index, and to adiposity as a factor in modifying the effects or possibly blunting the effects of vitamin D supplementation. Because if you don’t take that into account, you can actually misinterpret the vitamin D trial.”
