Coach K’s final home game: UNC turns Mike Krzyzewski’s Cameron Indoor Stadium finale into its own celebration

DURHAM, N.C. — This is what can happen when you try to plan a retirement ceremony with 9,000 of your closest friends, with the tricky caveat of including a basketball game. 

This was to be Mike Krzyzewski’s night. Duke staged one of the grandest send-offs college sports had ever seen. It was nostalgic. It was elaborate. It was quintessentially Duke. Which means a lot of people loved it and a lot of people hated it. Which means it was perfectly done. All of this created the biggest buildup, probably ever, to a regular-season college basketball game.

And then the greatest rivalry in American sports added to its legend with one of the more surprising upsets in its 102-year history. 

Unranked, bubble-team-supreme North Carolina 94, No. 4 Duke 81. A Saturday night stunner at Cameron Indoor Stadium. 

Krzyzewski’s home finale farewell was officially sabotaged by Hubert Davis’ upset-minded Tar Heels. Who were these guys, by the way? Not the same group that got pasted by 20 at home against Duke a month ago. That team scored 67 and was flat embarrassed. Saturday night’s squad averaged a terrific 1.25 points per possession and looked like a top-10 team in the country. North Carolina, an 11.5-point underdog, didn’t just beat Duke, it ran away with the game and ruined Krzyzewski’s valediction in appropriate fashion.

Seldom are the nights when these two undying nemeses come together and either team is overwhelmingly presumed to beat the other. Foolishly, most of us thought just that. 

UNC handed K an L and snatched a W so sweet, it immediately ranks near the top of the Tar Heels’ greatest regular-season victories in the 100-plus year history of this proud program.  

This is how you rivalry. 

“Hate to use the Navy term,” Krzyzewski, the West Point grad, said. “But keep a ship afloat. Our ship was going to sink today, no matter what.”

The postgame festivities at Cameron were to be a celebration of an unparalleled career. And they were, but that fête came with awkwardness. After the loss, Duke players walked out, sullen, and wound up sitting on the bench where UNC was just moments earlier. Krzyzewski reemerged with some understandable disappointment in his face. He held the hand of his beloved wife, Mickie, then leaned into a quick off-script commentary as he walked alone to the microphone staged at center court. 

“This isn’t part of the program,” he said. “This is impromptu by me. I’m sorry about this afternoon.” 

The place wanted to cheer despite the loss. Krzyzewski wasn’t having it.

“Everyone be quiet,” he said. “Let me just say, it’s unacceptable. Today was unacceptable, but the season has been very acceptable. And I’ll tell you, the season isn’t over.”

Then the cheers came. He wanted everyone to see that’s the guy who they needed to see in that moment. It’s who they saw most of the night, too, because North Carolina had Duke on its heels and Krzyzewski was vintage in his ire and fire. He rode the referees and looked nothing like a man ready to walk away from competition. This was Carolina, dammit. Afterward, he had nothing but compliments to pay to Davis’ players and program.

“Their program, like ours, is built on a lot of pride and they showed that today in coming here,” Krzyzewski said. 

With Dirk Nowitzki, Jerry Seinfeld, Adam Silver, Kyler Murray and many more luminaries in the building, it’s fair to guess Cameron Indoor Stadium had never been this filled. The official capacity is 9,314, but it’s likely more than that got into the arena. Cameron Indoor got hot, stayed hot, and that added thickness to an environment that was already heavy as soon as the Cameron Crazies filed onto the rickety bleachers.

At their opposite was esteemed Duke basketball alumni — nearly 100 of them, almost half of Krzyzewski’s former players — packed the bleachers a few rows behind the Duke bench, most of them standing for the entire game. 

“It has been emotional but before the game you’re thinking and you get a little teary-eyed and then you say, ‘Whoa, I can’t do that,’ ” Krzyzewski said. “And so then you go on the court and then you feel it. Actually, when all the players were there, it was just, like, wow, it wasn’t as emotional as it was great.”

Christian Laettner, Grant Hill, Jay Williams, JJ Redick, Shane Battier, Carlos Boozer, Steve Wojciechowski and nearly 90 of their Brotherhood brethren stood the entire game behind the Duke bench. It felt like the varsity team watching the JV early tip. These guys wanted a Carolina crunching. They wanted no drama. A UNC-Duke game without theatrics, without tension. The kind of game they seldom were afforded.

Yeah, right. 

“The last few days has been so really good and it’s been like a celebration of our program and because I’m stopping coaching, it’s kind of like a celebration of me, which I don’t like that,” Krzyzewski said. “So, basically we’re living in a penthouse the last few days with room service and everyone saying nice things and we didn’t play hungry today.” 

North Carolina was laser-focused, and it started within minutes of the Tar Heels’ overtime victory on Monday vs. Syracuse. Davis walked into the locker room and told the team there was no time to celebrate. They had less than five days to prepare for the biggest game of the season. Duke awaited. It was time to get to work and keep focus. Davis played the situation perfectly. North Carolina effectively prepped and practice in obscurity the entire week. It was reduced to a bit player in the buildup to Saturday, the game itself feeling secondary to the Krzyzewski farewell. 

Just what North Carolina wanted. Krzyzewski admitted afterward he could feel it coming, too.

“I was worried about a few things going into the game,” Krzyzewski said. “One: them.”

Intrigue arrived immediately. UNC — which never led in its Feb. 5 loss to Duke — opened the game with a 9-2 lead. Duke didn’t grab the lead until the 11:07 mark. The Heels withstood a 14-0 first-half Blue Devils run, steadying themselves enough to cut the halftime deficit to two. 

It turned out to be the first time this season North Carolina would win a game despite losing at the half. It was also historic in this matter: Never before had UNC played a game that saw four Tar Heels score at least 20 points. On Saturday, it happened: Armando Bacot 23, Caleb Love 22, RJ Davis 21 and Brady Manek 20.

Bacot was brilliant. The player Krzyzewski said was the best in the ACC this season went 10 for 11 from the floor. He also had a big block late that stymied Duke’s bid to keep it close. 

Saturday was also North Carolina’s first victory as an unranked team vs. a top-five-ranked Blue Devils squad at Cameron since 1990.

“They’re a really good team and they played a lot better than we did today,” Krzyzewski said.

This was Davis’ 31st game as North Carolina coach. It was, by far, his best. It was North Carolina’s most impressive performance of the season, without a close second. Amazingly, Davis rode his iron five the entire second half. Bacot, Manek, Love, Davis and Black were never subbed out. Late in the game, when you might’ve suspected one more Duke push was coming, instead it was David and Manek again and again. Trevor Keels’ 3-pointer from the corner with 2:58 remaining cut it to a 79-74 North Carolina lead. It’s the last time Duke would have hope for the win. Manek answered with a 3-pointer from the opposite corner, boosting the lead back to eight and keeping Duke at bay. 

UNC outscored Duke 21-10 over the final 5:12. Duke missed seven of its final 10 shots. Duke bested North Carolina 58.1{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} to 38.9{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} in first-half shooting. The second 20 minutes was a script-flip: UNC hit on 59.4{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809}; Duke withered with 42.1{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} shooting. Krzyzewski’s moment was melting away. Duke’s shots stopped falling, and all across Cameron faces began to droop, a stark contrast to the maybe-100 UNC fans squeezed behind the Tar Heels’ bench who began to pop with joy.  Outnumbered by 9,000-plus in this Gothic basketball cathedral, their time was coming. This was really going to happen.

After the game, this powerful scene in the locker room. Davis and Bacot embracing for more than a minute. Manek in tears, too. 

“He had some tough times at Oklahoma, and for him to be able to experience this side of it was huge,” UNC assistant Brad Frederick told CBS Sports. 

Making matters even better for UNC, the triumph doubles as a critical one for its NCAA Tournament résumé. Hard to see how the NCAA Tournament selection committee could keep this team out of the field, no matter what happens in the ACC Tournament. This team should be in.

As the clock expired, Carolina’s players convened on the court in blissful celebration. The Crazies were stunned. Love waved them goodbye before curling back into the handshake line. (And then we got a little extra rivalry saltiness, as Duke assistant Chris Carrawell didn’t shake hands with Davis, lighting up message boards and social media in the process.)

It was Krzyzewski’s day, then it was North Carolina’s night. The North Carolina bus pulled up to Franklin Street and stopped short of a police barricade around 9:30 p.m. Thousands were out in celebration. The players got out and joined Carolina students. They earned it. Even Davis was obliged to pop his head out and acknowledge the crowd. He has had a good year, and the fans haven’t always treated him that way. Maybe this night will turn things for the better.

Now the ACC Tournament awaits. Krzyzewski admitted the past three or four days had him a “little bit worn out.”

“I’m glad this is over,” he said. “Let’s just coach and see what the hell happens in the tournament. This has kind of been a surreal few days, and a big part of it occurred, I think, because we had already won (the ACC regular-season title). You don’t feel the pressure of we have to win that game.”

We could come to learn that the best thing for Duke was losing this game. The disappointment of this loss is probably never washing off (if there was one game in the regular season you couldn’t lose, it was this one), but the shock of it soon will. Perhaps that will bring Duke the same kind of urgency and focus that North Carolina deployed in Saturday night, when the best rivalry in American sports twisted the plot yet again.

Given the buildup and presumptive nature of a Duke win by plenty, it’s easily arguable this is the most gratifying — if not amusing — win for UNC fans over Duke ever. You get one chance to spoil Coach K’s final home game. Carolina passed the test. Its base will hold this over the Duke faithful forever. It’s so bitter for Duke, but that makes it tremendous for the rivalry. Down in Carolina, this one will live in conversation forever, no matter the favored shade of blue.

NFL reveals home teams for 2022 international games, plus Kyler Murray makes contract demand

Welcome to the Monday edition of the Pick Six newsletter!

Someone must have just told the NFL that it’s the last day of February, because it feels like the league tried to cram as much news as possible in today before the calendar flips to March. For one, the league revealed the five teams that will be hosting international games this year.

For 2022, not only will we have three London games and one game in Mexico City, but there will also be a game in Germany for the first time ever and the Buccaneers will be the home team. If the game is scheduled to be played during Oktoberfest, there is a 100{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} chance I will be volunteering to cover it, and if I get sent, there’s also a 100{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} chance I will be volunteering to taste every beer at Oktoberfest. 

Besides the international news, Kyler Murray also made some headlines on Monday by demanding a contract from the Cardinals even though he has two years left on his current contract. I feel like that situation is going to get a little awkward. 

We’ll be covering Murray and the international stuff today, so let’s get to the rundown.

1. Today’s show: Monday Mailbag!

During the NFL offseason, we like to spice things up each week on the Pick Six Podcast by adding a listener mailbag every Monday. 

Our mailbag episodes are pretty simple: We read listener questions during the show and then answer as many of those questions as possible. Questions can be about literally anything. I mean, I’m pretty sure there was one time where we spent 10 minutes debating the best national restaurant chain and all I’m going to say about that is that I live roughly two miles away from a Chilis and I eat there way more than I probably should.

Here’s one question that we answered in today’s mailbag: 

Q: Does defense still win championships? The past two Super Bowls seem to support that, but Kansas City’s Super Bowl win was fueled by an offensive resurgence in the fourth quarter. Should teams thus invest more in defenses?

A: If I were an NFL general manager — which will probably be my next career move after I retire from newsletter writing — I’d try to build a team that isn’t overly dependent on either side of the ball. If you build a team that relies completely on its offense and that offense disappears for one game in the playoffs, then you’re likely going to lose (The Chiefs’ offense in the second half of the AFC title game is exhibit A of that). If you build a team that’s completely reliant on its defense and that defense implodes in a game, you’re likely going to lose. The best example of this from 2021 is the Buffalo Bills. They had the best defense in the league by nearly every metric, but then that defense got torched by the Chiefs in the divisional round and Buffalo got sent home packing from the playoffs. 

One stat that has been a good indicator of playoff performance is point differential. In eight of the past nine years, the team that has won the Super Bowl has ranked in the top six in point differential in the regular season. To have a high point differential, you usually need an offense that can score and a defense that’s not getting lit up every week. I think what I’m trying to say is that defense can help you win championships, but it’s unlikely you’re going to win one with a team built completely around the defense (like the 1985 Bears), so going that route would be risky in this day and age where nearly every rule favors the offense. 

When I inevitably get that general managing job, I’ll probably invest 58{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} of my salary cap on offense and 42{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} on defense. Obviously, that leaves zero percent for special teams, so that’s something I’ll clearly have to figure out between now and when I get hired.

2. NFL reveals home teams for 2022 international games

The NFL will be playing a total of five international games in 2022 and we already know the home teams in each of those games because the league revealed them on Monday. The five international games will be tied for the most that the NFL has ever played in a single season. 

Here’s a look at the five teams that will be headed abroad: 

  • Buccaneers headed to Munich. The NFL’s first regular-season game ever in Germany will feature the Tampa Bay Buccaneers. This will be Tampa Bay’s fourth trip to Europe with the three prior games all being played in London. 
  • Cardinals headed to Mexico City. Back in 2005, the Cardinals became the first NFL team to play a regular-season game outside the United States when they hosted a game in Mexico City, and now, they’ll be making their return 17 years later. The Cardinals have been the NFL’s unofficial international guinea pigs over the course of their existence. Not only did they play in the first regular-season game outside of the U.S., but they also played in the first preseason game outside North America in 1976 (Japan) and the first preseason game ever in Europe in 1983 (London). 
  • Packers headed to London (Tottenham Hotspur Stadium). The NFL has been sending teams to London for 14 years and over that time, 31 of the league’s 32 teams had made the trip abroad. The only team that still hasn’t played in London is the Packers, but that drought will finally be coming to an end. 
  • Saints headed to London (Tottenham Hotspur Stadium). For the third time in franchise history, the Saints are headed to England. The Saints have a 2-0 all-time record in London with a win over the Chargers in 2008 and a victory over the Dolphins in 2017. If they moved all their games to London, they’d probably never lose again. I think that’s how that works. 
  • Jaguars headed to London (Wembley Stadium). No surprise here. The Jaguars are the closest thing the NFL has to a team being based in London. The Jags have gone to London in every season since 2013 and that streak will continue this year. 

We won’t know who each of these teams will be playing until the schedule comes out in the spring. One other notable aspect of the international games this year is that 2022 marks the beginning of a rotation that will see every NFL team play at least one international game every eight years. 

3. Kyler Murray wants a new contract

If someone you’re in a relationship with deletes you from one of their social media accounts, that’s usually a sign that maybe things aren’t going so well and that appears to be the case with Kyler Murray and the Cardinals. Less than three weeks ago, Murray deleted every Cardinals-related picture from his Instagram account and it appears we now know why: He wants a new contract.  

Murray’s agent, Erik Burkhardt, released a lengthy statement on Monday and I read the entire thing so that you don’t have to. 

Here are the details on the statement: 

  • Murray wants to stay in Arizona with a catch. “He absolutely wants to be your long-term quarterback. He desperately wants to win the Super Bowl,” the statement notes. However, the statement also notes that for the Cards to win a Super Bowl, it will take “incredible commitment” from the organization implying that maybe the team hasn’t shown that incredible commitment just yet. 
  • Burkhardt has already sent a contract proposal to the Cardinals. Murray’s agent noted in the statement that the QB wants a contract that, “Provides financial protection, is in-line with the current QB market that compares his results alongside relevant comps, lowers his 2022-23 salary camp number to allow the Cardinals to re-sign other deserving teammates and add additional free agents.”
  • Burkhardt basically called out the Cardinals to get a deal done. “Actions speak much louder than words in this volatile business. It is now simply up to the Cardinals to decide if they prioritize their rapidly improving, 24-year-old, already 2x Pro Bowl QB, who led the organization from 3 wins before his arrival to 11 wins and their first playoff appearance in 5 years, or rather, if they choose to financially prioritize committing to other areas and continuing to merely talk about addressing Kyler’s long-term future as their QB.” 
  • The Cardinals apparently haven’t responded to the proposal. According to Kyle Odegard, who used to write for the team’s official website, Murray’s camp sent in the contract proposal more than a week-and-a-half ago, but the Cards still haven’t responded. 
  • Murray is eligible for a new contract for the first time in his career. Under rules of the collective bargaining agreement, a first-round pick isn’t eligible for a contract extension until after playing three seasons, so this year marks the first time that Murray is eligible for an extension. That being said, teams don’t generally rush to make a deal with a QB who’s only been in the league for three years. No QB has ever gotten a deal before May 15 following his third season in the league.  Patrick Mahomes, Deshaun Watson, Josh Allen, Carson Wentz and Jared Goff all got extended after their third year, but those extensions didn’t happen until June or later. 
  • The Lamar Jackson factor. The Ravens QB has been much more successful in his NFL career, but he still hasn’t gotten an extension done with the Ravens. Murray might be rushing to get a deal done BEFORE Jackson because he knows that asking for more money than the Ravens QB would look borderline ridiculous considering that Jackson has an MVP and a playoff win under his belt, two things that Murray doesn’t have. 

Murray has put the Cardinals in an impossible spot and it will be interesting to see if they respond by giving him an extension right away or if they milk this thing out until later in the offseason. 

4. AFC North team needs: Bengals need to fix their two biggest weaknesses

Over the course of this week, we’ll be covering the offseason needs for each team by going through each division and today we’re going to be going over the AFC North. 

With that in mind, Bryan DeArdo went through every team in the division and made a list of their biggest needs to fill this offseason. 

BENGALS
Top-five needs (in no particular order)
: CB, C, OG, OT, LB
DeArdo’s take: Anyone who watched the Super Bowl saw the Bengals’ two biggest weaknesses. Expect the Bengals to address their offensive line in both free agency as well as during the NFL Draft. Speaking of the draft, our four CBS Sports NFL Draft experts have Cincinnati selecting either a lineman or a cornerback with the 31st overall pick.

BROWNS
Top-five needs: WR, DT, LB, C, EDGE
DeArdo’s take: With Jarvis Landry’s future uncertain, the Browns need to be proactive as it relates to adding to their receiving corps. Receivers who are slated to hit the market include Allen Robinson, Chris Godwin, Will Fuller, Jamison Crowder, T.Y. Hilton, JuJu Smith-Schuster, DJ Chark, Michael Gallup, Demarcus Robinson, and Richie James. Don’t be surprised if the Browns spent the 13th overall pick on a receiver. 

RAVENS
Top-five needs: OT, LB, EDGE, DT
DeArdo’s take: Baltimore’s inability to protect their quarterback and apply pressure on others were two of their biggest issues in 2021. The Ravens’ O-line, which played all but one game without Ronnie Stanley, finished 21st in Pro Football Focus’ end-of-season ranking. The Ravens also may be preparing for an overhaul on their defensive line, as Brandon Williams and Calais Campbell are entering free agency. Baltimore will surely be in the market for a pass-rush specialist when free agency begins. 

STEELERS
Top-five needs: QB, OT, OG, CB, DT
DeArdo’s take: Regardless of whom the Steelers’ quarterback is, they need to apply him with a strong offensive line and a formidable receiving corps. DJ Chark would be a nice complement to Diontae Johnson and Chase Claypool. Pittsburgh will likely draft a receiver as well as multiple offensive linemen. The Steelers will likely also use free agency to add to their offensive line.

5. Dolphins wanted Sean Payton (as coach) and Tom Brady (as a minority owner) in 2022

The Miami Dolphins had a grand plan for 2022 that involved bringing both Sean Payton and Tom Brady to South Beach, but like most things involving the Dolphins, none of it ended up working out.

Here’s what we know about the dream team that owner Stephen Ross was trying to build: 

  • Dolphins wanted to hire Payton. According to Pro Football Talk, after firing Flores in late January, Miami decided to call up the Saints to try to work out a deal for Sean Payton. The Dolphins made the call after Payton resigned, but before the Brian Flores lawsuit. The Dolphins never got to talk to Payton, though, because the Saints turned down Miami’s request to work out a trade. Although he’s out of football right now, Payton is currently under contract with the Saints, so if he does eventually want to return to the NFL with a different team, that new team will have to make a trade with New Orleans. 
  • Dolphins also wanted to bring in Tom Brady. The Dolphins were hoping to land Brady not as their QB of the future, but as a key minority owner. However, the team has put the idea of adding Brady on hold because there’s a chance he could return to the NFL. Once it becomes clear that he’s going to stay retired, the Dolphins will likely once again put on the full-court press and try to get him to sign on as a minority owner. 

Brady is still under contract with the Buccaneers, so any team that wants him to play quarterback would have to work out a deal with Tampa Bay. However, if a team wants him to join their ownership group, they wouldn’t have to work anything out with the Bucs. 

6. Rapid-fire roundup: Buccaneers hit with surprise retirement

It’s been a busy 24 hours in the NFL, and since it’s nearly impossible to keep track of everything that happened, I went ahead and put together a roundup for you. 

  • Buccaneers hit with surprise retirementThe Buccaneers were hit with a shocking retirement on Sunday when starting guard Ali Marpet decided to call it quits at the age of 28. Marpet was a key part of Tampa Bay’s offensive line and had been with the Buccaneers since 2015 when they took a huge risk by making him a second-round pick out of a D-III school (Hobart). The Buccaneers could be in for a total offensive rebuild this year. Not only are they losing Marpet and Brady, but they could also lose center Ryan Jensen and right guard Alex Cappa, who will both be free agents. 
  • Changes could be coming to NFL overtime. The Colts have submitted a proposal that would call for a change to the NFL’s overtime rules. Under Indy’s proposal, each team would get a possession in OT. This proposal will likely be voted on at the league meetings, which run March 27-30 in Florida. To become a rule, a proposal needs at least 24 of the league’s 32 owners to vote for it. 
  • Sean McVay passes up $100 million TV offer to keep Rams job. Sean McVay turned down some serious money to return to the Rams. According to the New York Post, Amazon was set to offer McVay a contract worth up to $100 million over five years. McVay confirmed on Friday that he would definitely be returning to the Rams for the 2022 season. He’s also in line to get a huge raise with the Rams expected to give him an extension “soon,” according to ESPN.com
  • Amazon now eyeing Kirk Herbstreit. With Amazon getting “Thursday Night Football” for the 2022 season, the internet giant has been busy looking for a color analyst. After getting turned down by Troy Aikman and Sean McVay, it appears Amazon is now going after ESPN’s Kirk Herbstreit. According to the New York Post, there’s a chance Herbstreit could end up doing NFL games for Amazon while still fulfilling his normal college obligations for ESPN. 
  • Bengals starter played in Super Bowl with torn labrum. Cincinnati linebacker Logan Wilson had surgery on a torn labrum last week. The shoulder injury actually caused him to miss three games this season, but he then returned in Week 17 for the Bengals’ huge win over the Chiefs. Wilson played a total of five games with the injury, including all four postseason games.  
  • Matt Corral won’t throw at the combine. One of the top quarterbacks in this year’s class has decided that he won’t be throwing at the combine this week. According to NFL.com, Corral made that decision because he’s still recovering from a sprained ankle that he suffered in the Sugar Bowl.

NFL announces international home marketing area teams and markets

This includes in-person and digital marketing, corporate sponsorship sales, fan events and activations, youth football activities, merchandise sales and co-marketing relationships with other sports and entertainment properties in the market.

Clubs can begin to activate in their awarded market(s) on January 1, 2022.

  • A team of researchers recently developed a novel way of delivering nucleic acid-based medicines using a method similar to an ancient cupping technique, Home Garden USA.

Over the past 20 years, scientists have developed a range of nucleic acid-based medicines as both treatments and vaccines.

Nucleic acids are present in all living cells, most notably as DNA and RNA.

Treatments and vaccines that use nucleic acids work by inhibiting, adding, replacing, or editing DNA and RNA in host cells.

How well these treatments and vaccines work depends on how well they can be delivered to the host’s cells. If directly injected into tissue, for example, most nucleic acids will rapidly degrade unless protected.

The Pfizer-BioNTech and Moderna COVID-19 vaccines are both nucleic acid-based medicines. Nucleic acids in these vaccines are enclosed and protected by lipid nanoparticles that deliver them across the host cell membrane.

Although they are effective, drugs delivered in this way are vulnerable to degradation during storage.

One delivery method for nucleic acid-based medicines is electroporation. This involves delivering nucleic acids to cells made more permeable using a pulse of electricity.

While somewhat effective, these electrical impulses can cause muscle contractions, tissue damage, and pain, and they may not be suitable for people who use implantable electrical devices such as defibrillators or pacemakers.

Using this method also requires substantial training, and equipment costs may make it inaccessible to regions and populations with fewer resources.

Creating a method for delivering nucleic acid-based medicines that overcomes unwanted side effects and cost barriers while still being effective could increase the accessibility of vaccines to areas with limited resources.

Recently, researchers from Rutgers, the State University of New Jersey in Piscataway, and GeneOne Life Science in Seoul, South Korea, developed a method based on an ancient cupping technique to deliver nucleic acid-based medicines.

“Development of enhanced delivery technologies plays an instrumental role in bringing nucleic acid-based biologics to broad use and clinical relevance, and worldwide vaccine distribution is just one example,” said senior study author Prof. Hao Lin.

“We have demonstrated an alternative, safe, and effective transfection platform that yields high levels of transgene expression.”

“The advantages also include device cost effectiveness, […] manufacturing scalability, and minimal requirements for user training,” he added.

“Because of the inherent advantages of DNA, not least of which is avoiding cold-chain requirements of other vaccines, this technology facilitates vaccination programs into remote regions of the world where resources are limited,” he added.

The study now appears in the journal Science Advances.

Cupping therapy is an ancient treatment method from the Middle East and China. It involves applying heated cups to the skin to create negative pressure and increase blood circulation to promote healing.

Since 1950, doctors have used the method in hospitals in China and elsewhere for conditions such as acne, facial paralysis, and dyspnea.

Although evidence of its health benefits is lacking, the authors of the new study decided to investigate a similar technique as a potential delivery method for nucleic acid-based medicine.

To test the method, the researchers gathered two groups of rats and injected both with DNA in the upper layers of their skin. One group received the “suction cup treatment” on their injection site, whereas the other group did not.

The researchers monitored the activity of the DNA with fluorescence microscopy. They found that gene expression from the vaccine was detectable at 4 hours post-vaccination. In the rats who also underwent cupping, however, gene expression was detectable at 1 hour post-vaccination.

At 24 hours post-injection, the researchers saw that gene expression was detectable eight times deeper under the skin among the rats who underwent cupping than among those who received DNA injections alone.

To investigate whether or not this technology could work to deliver potential COVID-19 vaccines, the researchers next injected the rats with a synthetic SARS-CoV-2 DNA candidate vaccine.

They split the rats into three groups:

  • The first group received two injections of the vaccine candidate without suction on days 0 and 14.
  • The second group received one vaccine, followed by dermal suction on day 0.
  • The third group received two vaccinations and dermal suction on days 0 and 14.

The researchers collected blood samples from the rats on days 0 and 14, before vaccination, and again on day 29.

They found that the rats who received cupping after the vaccine had a significantly stronger immune response than those who received the vaccine alone.

The researchers also found that immune responses in the rats who received a single vaccine followed by suction were not statistically different from those who received two vaccines followed by suction.

They also note that the rats who underwent suction had no evidence of tissue damage or lymphocyte infiltration in their injected areas.

The researchers say that the underlying mechanisms for suction-enhanced delivery are not clear. They hypothesize, however, that it may work via the CG pathway. This plays a role in determining which molecules travel through membranes and enter cells.

The researchers explain that it will be challenging to confirm the underlying mechanisms of cupping due to the complexity of the processes involved and a lack of availability of drugs used for testing.

Nevertheless, the researchers say that even without mechanistic understanding, it is still possible to improve the method.

“Currently, the procedure is two-step: an injection followed by suction,” Dr. Lin told Medical News Today.

“We are working on a combination device/method to render one-step application — that is, injection and suction in a single step by a single device,” he continued. “Furthermore, we are trying to reduce suction time even further to increase patient and staff acceptance.”

When asked whether this method could work for COVID-19 vaccines, Dr. Lin said, “Theoretically, this may be possible, but we cannot answer without testing since, except for DNA vaccines, COVID-19 vaccines are delivered into the muscle, whereas this device delivers [the] vaccine to the upper layers of the skin.”

“The [suction] device is being tested currently in phase 1 and phase 2 clinical trials of a COVID-19 vaccine. The documentation to enable filing with regulatory agencies for general and widespread clinical use is being readied,” he said.

The researchers conclude that cupping is a promising method for nucleic acid-based medicine delivery. However, further research is necessary to see whether it could work to deliver COVID-19 vaccines to humans.

“The suction device has numerous advantages relative to other devices for DNA vaccine delivery. First, it is very inexpensive and easy to use, requiring minimal training,” Dr. Lin told MNT. “Secondly, the device is run from a battery that is easily rechargeable.”

“Third,” he continued, “is the high degree of patient acceptance, as the device will help deliver DNA vaccines without pain or discomfort.”

“And fourth, unlike the significant setup times with other devices, this only requires changing out a cap and then pressing the power button; the machine will turn off in a few seconds.”

“This, coupled with the many advantages of DNA as a vaccine type — the greatest advantages being the ability to completely avoid the need for freezers from the clinician’s perspective, and low side effects and discomfort from the patient’s perspective — makes this an ideal vaccine platform for resource-limited and remote regions,” he concluded.
