Study links air pollution to higher risk of stroke and related death

New research from the Sun Yat-sen University School of Public Health in Guangzhou, China, amplifies how air pollution could affect the trajectory of health from stroke to subsequent death.

The study, recently published in Neurology, focused on the health impacts of PM 2.5 granules. This fine particulate matter (PM) is harmful to human health and has a diameter of fewer than 2.5 microns, about 30 times smaller than a single human hair.

Researchers also looked at nitrogen dioxide (NO₂) and nitrogen oxides (NO𝑥) levels in polluted air and measured air pollutant levels by their weight in micrograms — 1-millionth of a gram — per 1 cubic meter of air, expressed as μg/m3.

The study shows that for every PM 2.5 increase of 5 μg/m3, the risk of a first stroke rose by 24{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809}, and the risk of a first fatal stroke increased by 30{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809}.

The pollutants also caused a slight increase in the risk of cardiovascular mortality for people who have already had a stroke. Dr. Franco Folino, Ph.D., research cardiologist in the department of Cardiac, Thoracic and Vascular Sciences at the University of Padova in Italy, not involved in the study, told Medical News Today:

“Despite the many studies that have shown the harmful effects of pollution on health, there still seems to be little awareness on the need to take adequate measures to reduce exposure to pollutants, particularly in populations at greater risk.”

The observational study is based on an analysis of the health records of 318,752 people in the UKBiobank and several years of UKBiobank air pollution data.

The researchers tracked the health progress of these individuals for 3,765,630 person-years of follow-up. During that time, 5,967 people experienced first or incident strokes. In addition, 2,985 individuals had post-stroke cardiovascular effects, and there were 1,020 subsequent deaths.

The researchers also used 1 year of air pollution data to estimate exposure based on where people lived. They modeled each individual’s exposure to pollutants based on land use, traffic, population, and topography data.

The data show that people who had strokes during the study had an average exposure of 10.03 µg/m3 of PM 2.5. For people who did not have a stroke, the exposure was 9.97 µg/m3.

“This new study tries to evaluate the ‘dynamic’ effects of pollution on stroke risk and mortality in a large population during a very long follow-up period,” Dr. Folino said.

“In essence, this is another demonstration of the long-term harmful effects of pollution, and in particular of fine particles and nitrogen oxides, on [the] cardiovascular system.”

Outdoors, PM 2.5 comes mostly from fossil-fuel-burning vehicles such as cars, trucks, buses, traditional trains, snowmobiles, and construction equipment. Any process involving burning wood, oil, gasoline, or coal may also contribute, including power plants.

In addition, these small yet dangerous particles can travel in the air, so natural events such as forest fires and volcanos distribute them over long distances.

Inside the home, PM 2.5 may come from cigarettes, burning candles, cooking on a stove or in an oven, fireplaces, and space heaters that burn fuel.

“When we consider air pollution, we do not have to think only of the quality of air we breathe outdoors. Very often, pollution can be greater at home,” Dr. Folino said, adding that people could also consume PM 2.5 in water and food.

“PM 2.5 seems particularly harmful because its small size allows it to enter more deeply into the smallest bronchi, where it can induce the inflammatory phenomena that seem to be largely responsible for the harmful effects of particulate matter.”

“All people, but especially those with a high cardiovascular risk, should avoid exposure to high pollution levels,” Dr. Folino said. “We have to carry out physical activities in healthy areas and pay attention to the possible polluting sources inside [the] home.”

Of course, Dr. Iyer noted that people living in densely populated urban areas aren’t necessarily able to avoid air pollution.

“Take care of your health in other ways to make sure that you have strong lungs by exercising, staying physically fit, having a good diet, and not smoking. Those are all things to mitigate and offset the exposure to particulate matter, air pollution.”

However, there has been little research exploring the ways in which the distribution of our daily energy intake may influence long-term cognitive health, and whether it has any impact on the risk of developing dementia.

According to the World Health Organization (WHO), around 55 million people worldwide have dementia, with 10 million new cases being diagnosed each year. As the world’s population is aging — the proportion of older people is rising steadily — the WHO estimates 78 million people will have dementia by 2030, and 139 million by 2050.

To better understand the effects that energy intake and meal timing have on cognition, a new study takes a look at the potential effect on cognitive decline of different meal schedules, or temporal patterns of energy intake (TPEI).

The results show that consuming three balanced meals each is associated with better cognitive function, compared to other, less evenly distributed ways of consuming one’s total energy intake, or TEI.

“To our knowledge, this study is one of the few population-based studies that explore the association of TPEI and cognitive decline, although accumulating studies have linked TPEI to health outcomes, including obesity, hypertension, and cardiovascular health,” the authors wrote.

The study also demonstrates that skipping breakfast is associated with worse cognitive function and faster cognitive decline.

The study was recently published in Life Metabolism.

 

The researchers drew their conclusions from an analysis of data from the 1997–2006 China Health and Nutrition Survey.

Included in that data were the meal habits of 3,342 people in China for whom the survey had collected up to four repeat entries over 10 years. Individuals were at least 55 years of age, with the average age being 62.2.

The researchers categorized individuals’ meal timing into six eating patterns:

The breakfast-skipping pattern was linked to a cognitive decline of 0.14 cognitive-test points a year, compared to the evenly distributed pattern.

No other similar declines were seen for other patterns.

Dr. Clifford Segil, a neurologist at Providence Saint John’s Health Center in Santa Monica, CA, not involved in the study, described this finding to Medical News Today as “fascinating.”

Previous research has indicated that meal timing affects the body’s circadian clock. In mammals, the circadian clock resides in two clusters of nerve cells called the suprachiasmatic nuclei (SCN), which are located in a region at the base of the brain called the anterior hypothalamus.

Dr. Hoon-Ki Sung, Ph.D., associate professor in the Department of Laboratory Medicine & Pathobiology at the University of Toronto, explained to MNT:

Other studies have linked meal timing to short-term cognitive function improvement.

The Western three-meals-a-day schedule grew out of the needs of employers and workers during the Industrial Revolution. Before that, two large meals a day, based on household and farming tasks, were more common.

“I think common sense says you should eat a meal before the time of the day that you’re going to be the busiest,” added Dr. Segil. “Some people are busy in the morning, and that’s why a big breakfast [is often] advised, especially for school-age kids.”

Both genetic and environmental factors influence stroke risk. Managing cardiometabolic risk factors and promoting healthy lifestyle behavior are frontline strategies for improving cardiovascular health and decreasing stroke risk.

It has been unclear whether improving cardiovascular health may offset the genetic risk for stroke.

The study appears in the Journal of the American Heart Association.

“The public message is clear,” Dr. Tatjana Rundek, professor of neurology and public health sciences at the University of Miami, not involved in the study, told Medical News Today.

“Regardless of the potential of harboring ‘bad’ genetic risk, improving cardiovascular health should be the most important priority for public health. Promoting ideal cardiovascular health should start at an early age, and many of us believe that we should start with a healthy diet and exercise at birth,” she noted.

For the study, the researchers analyzed data from 11,568 middle-aged adults who were stroke-free at baseline, and followed them for an average of 28 years.

Their lifetime risk of stroke was estimated from levels of genetic risk based on a validated stroke polygenic risk score and levels of cardiovascular health according to the American Heart Association’s “Life’s Simple 7,” which are now revised and updated to “Life’s Essential 8.”

The initial “Life’s Simple 7” recommendations are:

  • cholesterol control
  • blood pressure control
  • blood glucose control
  • physical activity
  • healthy diet
  • no smoking
  • maintaining a healthy body mass index (BMI).

Participants were assessed for “Life’s Simple 7” at the start of the study from a mix of self-reported and clinically-assessed measures.

Over the follow-up period, 1,138 participants were diagnosed with stroke. Altogether, they found that participants with the highest genetic risk and lowest “Life’s Simple 7” scores had the highest lifetime risk of stroke at 24.8{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809}.

They further found that across all polygenic risk score categories, those with an optimal “Life’s Simple 7” score had a 30-43{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} lower lifetime risk of stroke than those with an inadequate “Life’s Simple 7” score.

This, they noted, corresponded to 6 additional years of stroke-free life in those with the highest genetic risk.

Prof. Lu Qi, distinguished chair and professor in the Department of Epidemiology at Tulane University School of Public Health and Tropical Medicine, not involved in the study, told MNT:

“‘Life’s Simple 7’ [has] been related to lower genetic risks of cardiovascular diseases including stroke in previous studies. It is not surprising the optimal ‘Life’s Simple 7’ score is associated with a lower genetic variation associated stroke risk.”

When asked how “Life’s Simple 7” might reduce genetic stroke risk.

Prof. Rundek said that “[t]he exact mechanism by which the combined risk/ lifestyle factors and genetic factors affect the risk for stroke is unknown and likely complex.”

“One way of explaining how ideal cardiovascular health — ‘Life’s Simple 7’ — can reduce genetic stroke risk is to think about genetic susceptibility to stroke risk in the presence of deleterious ‘Life’s Simple 7’ factors, because certain genes could be expressed only when activated by the presence of environmental factors or poor ‘Life’s Simple 7’ [scores for] cardiovascular health,” she noted.

“If we reduce these environmental factors and achieve ideal ‘Life’s Simple 7’ cardiovascular health [score] — stroke risk genes that we potentially harbor — would not be expressed to do harm and contribute to increased stroke risk,” added Prof. Rundek.

The researchers concluded that maintaining optimal cardiovascular health can partially offset a high genetic risk for stroke.

When asked about the study’s limitations, Prof. Qi noted that as the study was observational in nature, it is “limited for causal inference.”

Prof. Christie M. Ballantyne, chief of Cardiology at Baylor University, also not involved in the study, further pointed out that:

“The data in African Americans was not robust, and other racial and ethnic groups, such as Hispanic, South Asian, and East Asian, were not well-represented in this study. Additional studies in other populations are needed to optimize the polygenic risk scores to be more useful in clinical practice for all of our patients.”

Prof. Rundek added that “[i]t may be hard to achieve and maintain ideal ‘Life’s Simple 7’ cardiovascular [score] if there is a strong individual genetic susceptibility to stroke risk [which includes] an increased risk of hypertension and other ‘Life’s Simple 7’ factors.”

“In addition, there are certain genetic markers — rare alleles — that are not included in polygenic risk scores as they contribute to risk by only a small amount. “An ICD is capable of terminating ventricular arrhythmias by anti-tachycardia pacing (ATP), shock, or both. ATP is used for VT and tries to pace the heart faster than the arrhythmia in order to block the pathological circuit, take the lead of the heart rhythm, and then slow down the pacing until a normal heart rate is obtained,” she said.
