A decline in certain cognitive abilities, such as memory and attention, is typical with aging. However, some individuals may experience dementia, which involves a severe decrease in cognitive abilities that impair daily functioning.

Studies show that individuals who exercise regularly have a lower risk of Alzheimer’s disease and all-cause dementia. Moreover, physical activity can slow down the progression of cognitive decline.

Scientists do not fully understand the mechanisms through which physical activity produces these cognitive benefits in humans.

A recent study led by researchers at the University of California San Francisco (UCSF) shows that reducing inflammation in the brain may mediate the cognitive benefits of physical activity.

Specifically, the researchers found that physical exercise had associations with reduced activation of microglia, the primary immune cells in the brain.

The study’s co-author, Dr. Kaitlin Casaletto, a professor at UCSF, told Medical News Today, “Many studies show that physical activity relates to better brain and cognitive health (e.g., estimates indicate that inactivity alone accounts for 13{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} of Alzheimer’s disease cases worldwide). Yet, we still do not fundamentally understand the mechanisms linking physical activity to cognition in humans. Our study is the first human data showing that microglial activation (“brain inflammation”) may be a meaningful mechanism.”

The study appears in the Journal of Neuroscience.

The nervous system consists of two major cell types: neurons and glial cells. Neurons are primarily involved in transmitting electrical and chemical signals, whereas glial cells protect and support neurons. More recently, scientists have discovered that glial cells can modulate signal transmission between neurons.

Animal studies suggest that the glial cells in the brain may mediate the beneficial effects of physical activity on cognitive function. Specifically, physical activity is known to alter the activity of microglia, a sub-type of glial cells.

Microglia are the brain’s immune cells and become activated in response to an infection or neuron damage.

Activation of microglia can benefit the immune system as it mounts an inflammatory response against an infection. But an abnormal increase in the activation of microglia can damage neurons.

Chronic low-grade inflammation in the brain is a characteristic of aging and neurodegenerative disorders such as Alzheimer’s disease. Moreover, studies show that these conditions involve an abnormal increase in the number of activated microglia in the brain.

Scientists know that physical exercise in animals reduces the activation of microglia and other brain markers of inflammation.

Microglia can also modulate the structure and function of synapses, which are specialized contact sites through which neurons communicate with each other. Microglia play an important role in the formation and elimination of synapses.

Moreover, they can modulate the strength of these synapses, thus influencing signal transmission between neurons.

Studies in animals show that the cognitive benefits of physical activity have associations with improvements in synaptic health or integrity. Furthermore, these studies suggest that microglia may mediate the effects of physical activity on synaptic integrity and cognitive function.

The present study investigated the relationship between physical activity and microglial activation in older adults. Given the association between physical activity and improvements in cognitive function and synaptic health, the study estimated the extent to which changes in microglial activity may support these effects of physical activity.

The study found that, as in animals, physical activity had associations with reduced microglial activation in older adults. Moreover, the study’s results suggest that reduced microglial activation could be one of the brain pathways through which physical activity protects individuals from cognitive decline, especially in Alzheimer’s disease.

The present study consisted of 167 deceased individuals enrolled in the Rush Memory and Aging Project (MAP). The Rush MAP is a longitudinal study that aims to identify risk factors associated with the development of Alzheimer’s disease.

The Rush MAP includes older adults without dementia at enrollment and involves annual assessments for dementia risk factors. The participants in the project had agreed to donate their brains and other organs for post-mortem analysis.

The present study consisted of individuals with an average age of 87 years at the time of the first physical activity test and 90 years at their death.

The researchers assessed daily physical activity using a wearable sensor called actigraph. Actigraphy provides an objective measure of physical activity by continuously tracking periods of motor activity and rest over multiple days.

In the current study, the researchers conducted actigraphy assessments continuously for up to 10 days. They also conducted yearly tests to assess cognitive function and the ability of the participants to perform various motor tasks.

After the participants’ death, the researchers analyzed the brain tissue to determine the number of activated microglia in four brain regions. They also assessed the levels of proteins associated with synaptic health and brain markers for Alzheimer’s disease, Lewy body dementia, stroke (infarcts), and other conditions.

The researchers found that higher physical activity levels measured using actigraphy had associations with a lower proportion of activated microglia when they considered all four brain regions together.

Factors such as limited motor function and cognitive impairment could potentially restrict the ability of the participants to engage in physical activity.

Consequently, the researchers adjusted their analysis for age, sex, motor, and cognitive function. They found that the association between the proportion of activated microglia and physical activity was independent of these variables.

The researchers then examined this association in individual brain regions. They found that the association between higher physical activity levels and reduced microglial activation reached statistical significance only in two brain regions — the ventromedial caudate and the inferior temporal gyrus.

Furthermore, the relationship between physical activity and reduced microglial activation was stronger in individuals with higher levels of brain pathologies in these two brain regions.

The brain pathologies found in the ventromedial caudate and inferior temporal gyrus consisted of microinfarcts (or mini-stroke) and Alzheimer’s disease-related pathologies, respectively.

In other words, individuals with higher levels of brain pathologies who regularly engaged in physical activity showed lower microglial activation than their counterparts with similar brain pathology levels but lower physical activity levels.

These data suggest that the effects of physical activity on microglial activation were specific to certain brain regions. These results are consistent with data showing that microinfarcts and brain pathologies associated with Alzheimer’s disease tend to be more common in the two brain regions.

The researchers then examined the association between microglial activation and clinically meaningful markers for dementia, namely cognition and the integrity of the synapses.

Microglial activation in the inferior temporal gyrus, but not the ventromedial caudate, had associations with a decline in cognitive functioning and lower levels of synaptic health markers.

Next, the researchers examined the extent to which reduced microglial activation associated with physical activity could improve cognition and the integrity of synapses.

Using a statistical method called mediation analysis, the researchers estimated that the decrease in the proportion of activated microglial in the inferior temporal gyrus contributed to over 30{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} of the effects of physical activity on cognition and synaptic markers.

Significantly, changes in microglial activation in the inferior temporal gyrus in individuals with higher levels of Alzheimer’s disease-related brain pathologies mediated more than 40{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} of the effects of physical activity on cognition and synaptic health.

In contrast, in individuals with lower Alzheimer’s disease-related pathologies, changes in microglial activity contributed to only 10{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} of the effects of physical activity.

Noting the significance of these findings, Dr. Tristan Qingyun Li said:

”This current work by Casaletto et al. is uniquely significant in that it provides the first evidence in humans to show that changes in microglial activation may be the mechanism bridging the beneficial effects of physical activity and healthier brain function. Furthermore, it points to a specific brain region, namely inferior temporal gyrus, that might be the most relevant for future microglia-based interventions.”

Dr. Li is a professor at the Washington University School of Medicine and was not involved in the study.

One of the study’s strengths included the use of actigraphy, which provides an objective assessment of physical activity levels. This is in contrast to other studies that often estimate physical activity levels using self-reports, which are prone to biases and inaccuracies.

This is the first human study to show that physical activity may improve cognitive functioning by reducing microglial activation.

Dr. Casaletto noted that the study had a few limitations. She said, “A major limitation of this work is the observational design. We cannot determine the directionality of effects, and it is likely that at least some of the relationship between physical activity and brain inflammation is bidirectional (i.e., brain inflammation leading to reductions in physical activity).”

Dr. Casaletto said that her research group intends to address this shortcoming in subsequent studies. She said, “We have a physical activity intervention study ongoing in which we hope to capture complementary markers of in-vivo inflammatory markers to help support causality of effects.”

Among other limitations, Dr. Casaletto noted, “We captured physical activity and cognition in life but brain inflammation and pathology at death. These are likely dynamic processes, and understanding the temporal link between lifestyle behaviors and biological changes is needed.”

Lastly, Dr. Casaletto noted that the study participants were primarily white and from Northeastern Illinois. Thus, the researchers do not know whether they can generalize the findings to a diverse population.

Are fasting-mimicking diets the future of cancer treatment?

  • A recent study investigated the safety, tolerability, and metabolic- and immune-related effects of fasting-mimicking diets (FMDs) in participants with cancer.
  • The results demonstrated that metabolic and immune changes were consistent with improved cancer outcomes, but they did not prove effectiveness.
  • More research is necessary to determine whether FMD-induced metabolic and immune effects improve the efficacy of standard cancer treatment and reduce mortality.

Changes in the types and amount of food a person eats can have positive effects on health and aging. Some evidence suggests that prolonged fasting may activate protective cellular mechanisms.

Scientists believe that these effects are partially due to temporary reductions in glucose, insulin, and insulin-like growth factor 1 (IGF-1). IGF-1 is a hormone that regulates growth, controls skeletal muscle metabolism, and rebuilds muscles in the human body.

As IGF-1 regulates cell growth and division, cell death, and cell survival, it may also promote the growth of cancerous tumors.

Preliminary studies with periodic cycles of prolonged fasting demonstrated antitumor effects, enhanced response to anticancer therapy, and protection of healthy cells from chemotherapy-induced damage.

However, complying with prolonged fasting and severe calorie restrictions may be challenging and have safety issues.

With this in mind, scientists have developed a “fasting-mimicking diet (FMD),” which is low in calories, sugars, and protein. The aim is to achieve effects similar to those that animal studies have shown to occur through fasting while preventing nutrient deficiencies.

The latest study is the first trial to investigate FMD in people with cancer.

The findings appear in the journal Cancer Discovery.

Lead author Dr. Claudio Vernieri, Ph.D., a medical oncologist and director of the Metabolic Reprogramming in Solid Tumors program at the FIRC Institute of Molecular Oncology (IFOM), explained to Medical News Today:

“Based on […] striking preclinical data, we decided to test if a severely calorie-restricted dietary regimen, which we specifically designed for use in oncology, could be well-tolerated by cancer patients and if [it] was sufficiently potent to modulate relevant metabolic and immunologic parameters.”

The researchers gave 101 participants with cancer an identical FMD for 5 days. The participants consumed up to 600 calories on day 1 and up to 300 calories on days 2–5. They followed this period of caloric restriction with 16–23 days of refeeding to regain the lost weight. The individuals then repeated these regimens every 3 or 4 weeks for up to eight cycles.

On average, participants went through four of these FMD cycles.

The study included people who had different types of cancer in various stages and were taking different anticancer treatments. It excluded individuals who were underweight or had a high risk of malnourishment.

In the 99 participants whom the team assessed, FMD reduced serum glucose levels by 18.6{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809}, serum insulin by 50.7{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809}, and serum IGF-1 by 30.3{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809}. These results were independent of the type of cancer, anticancer treatment, and stage of cancer.

The researchers assessed insulin levels because evidence suggests that this hormone stimulates cancer cell growth. Similarly, some evidence suggests that higher serum glucose levels also promote cancer progression.

After 5 days, the blood samples of 38 FMD-treated participants showed a significant decline in specific white blood cells called myeloid cells that are associated with immune suppression. They also had increased numbers of cytotoxic T cells, immune cells that can kill cancer cells. Both of these changes were independent of anticancer treatment.

The researchers then focused on a subset of 13 participants who had advanced triple-negative breast cancer and were receiving first-line cytotoxic chemotherapy (ChT). They compared immune changes between those receiving both ChT and FMD and 13 participants receiving ChT alone.

In the dual treatment group, the scientists measured similar immune effects: decreases in immunosuppressive myeloid cells and increases in activated CD8+ T cells, which are a type of cytotoxic T cell. In the group that took ChT alone, these immune changes were not present.

The study also reported an interim analysis of immune responses to FMD in 22 people with early stage breast cancer awaiting surgery. These participants started FMD 12–15 days before their surgery.

In these individuals, FMD produced similar metabolic changes, namely decreases in blood glucose, insulin, and IGF-1. The researchers also observed significant immune changes, which indicated the activation of an anticancer environment at the tumor level.

Dr. Vernieri told MNT, “The most relevant finding of the study is that our fasting-mimicking diet regimen boosts the immune system by activating specific immune cells that are clearly recognized as crucial in recognizing and killing cancer cells.”

One of the study’s strengths is that FMD safely produced the desired anticancer metabolic and immunologic changes across all study participants, regardless of the type of cancer, cancer severity, and anticancer treatment.

“The main limitation of our study,” Dr. Vernieri explained, “is the fact that the heterogeneity of enrolled patients does not allow [us] to conclude about the antitumor efficacy of the fasting-mimicking diet.”

MNT also spoke with Dr. Steven Vasilev, who was not involved in the study. Dr. Vasilev is an oncologist and medical director of Integrative Gynecologic Oncology at Providence Saint John’s Health Center in Santa Monica, CA. He is also a professor at Saint John’s Cancer Institute, which is also in Santa Monica.

Speaking about the challenges in interpreting the results, he said:

“You’re […] essentially trying to starve someone for some period of time, which leads to an incredibly complex immunological and metabolic response. Then [you’re] trying to figure out which [factor] is […] the one that’s responsible for returning tumor growth. And the answer is: It’s probably all of them, but you [don’t] know which one is the most essential.”

Dr. Vasilev explained that many cancer patients are already malnourished. For safety and ethical reasons, the current study did not include participants at high risk of malnourishment. This exclusion introduces bias, albeit necessary bias.

He also outlined a number of clinical limitations that need ironing out before doctors could use caloric restriction to treat cancer patients. For instance, he suggested that reducing calorie intake might have a negative effect on wound healing.

However, Dr. Vasilev also reiterated that much more research is necessary before this type of intervention enters the clinic.

Dr. Vernieri spoke with MNT about the next steps, saying, “Additional trials must be conducted in well-selected patient populations to clarify in which clinical contexts fasting-mimicking diet could be combined with standard anticancer therapies to improve their antitumor activity and efficacy.”

“In this perspective,” he continued, “we are conducting the phase 2, randomized trial BREAKFAST to investigate if combining severe calorie restriction with standard preoperative chemotherapy is capable of improving tumor responses in patients with limited-stage triple-negative breast cancer.”

He added, “I would like to highlight that to guarantee the safety of calorie restriction, patients should be managed by multidisciplinary teams with specific expertise in the field of fasting in oncology, and only in the context of clinical trials.”

Jonah Goldberg and Stephen Hayes resign from Fox News, protesting ‘irresponsible’ voices like Tucker Carlson

Every month or so, while conversing with sources at Fox News, I express surprise that Jonah Goldberg and Stephen Hayes are still employed by the network. After all, the two men are reality-based conservative thinkers who refuse to capitulate to Donald Trump. Unfortunately, Fox viewers rarely get to hear from them. They are booked by the network’s producers so rarely that their contracts could be likened to golden handcuffs.

Now they are ditching the cuffs.

Hayes and Goldberg announced Sunday night that they have resigned from Fox. The pair wrote in a blog post for The Dispatch, their online home, that Tucker Carlson’s “Patriot Purge” propaganda film was the last straw.
“Fox News still does real reporting, and there are still responsible conservatives providing valuable opinion and analysis,” the men wrote. “But the voices of the responsible are being drowned out by the irresponsible,” and Carlson is the case in point.
CNN and other news outlets have described how “Patriot Purge” is disturbing evidence of right-wing radicalization, complete with January 6 denialism and paranoid descriptions of a “new war on terror” targeting Republicans, with references to Guantanamo Bay and images of waterboarding.

“This is not happening. And we think it’s dangerous to pretend it is,” the Goldberg and Hayes wrote. “If a person with such a platform shares such misinformation loud enough and long enough, there are Americans who will believe — and act upon — it.”

Goldberg and Hayes simply could not be a part of it anymore.

New York Times media columnist Ben Smith, who broke the resignation news on Sunday, called up Carlson for reaction, and the trollish host naturally celebrated, saying “our viewers will be grateful” that Goldberg and Hayes are gone. But the conservative movement will suffer for it.
Rep. Liz Cheney, one of the Republican party’s foremost critics of Trump and the Big Lie, tweeted out a thank-you to the two men “for standing up for truth and calling out dangerous lies.”

A rare view inside Fox

Goldberg, in a phone interview with Smith, provided a rare on-the-record peek inside Fox’s political positioning. Per Smith, “Goldberg said that” he and Hayes “stayed on at Fox News as long they did because of a sense from conversations at Fox that, after Mr. Trump’s defeat, the network would try to recover some of its independence and, as he put it, ‘right the ship.'” But Fox’s decision to promote and publish “Patriot Purge” in early November was “a sign that people have made peace with this direction of things, and there is no plan, at least, that anyone made me aware of for a course correction,” Goldberg said.

In Sunday night’s post on The Dispatch, the two commentators said the “Patriot Purge” web series “creates an alternative history of January 6, contradicted not just by common sense, not just by the testimony and on-the-record statements of many participants, but by the reporting of the news division of Fox News itself.”

Hayes and Goldberg praised the “news side” (which keeps shrinking at Fox) but acknowledged that the opinion side has radicalized. “The release of ‘Patriot Purge’ wasn’t an isolated incident,” they wrote, “it was merely the most egregious example of a longstanding trend.” So they’re out. “We do not regret our decision,” they concluded, “even if we find it regrettably necessary.”

Monster Beverage (MNST) Exploring Merger Deal With Constellation Brands (STZ)

Energy drinks maker Monster Beverage (NASDAQ: MNST) is exploring a merger deal with Constellation Brands (NYSE: STZ), according to a report from Bloomberg news. The exact structure of the potential merger is yet to be available to the general public. Of course, a deal between these two beverage giants could be interesting. But, investors shouldn’t speculate on such rumors as it is still unclear if the deal would eventually materialize. 

Despite the supply chain challenges, Monster Beverage reported record third-quarter sales earlier this month. The energy drinks maker, which has Coca-Cola (NYSE: KO) as its major shareholder, will need to assess if such a merger could bring any positive synergy. Right now, it’s unclear if such a deal could jeopardize Monster’s exclusive distribution agreement with Coca-Cola. And if it does, it could affect Monster’s international growth ambitions.

Ford (F), Rivian (RIVN) Cancel Plans To Jointly Develop An EV

Ford (NYSE: F) and Rivian (NASDAQ: RIVN) no longer plan to co-develop an electric vehicle, the companies confirmed Friday evening. The two companies initially announced development of a joint vehicle when Ford invested in the EV start up in 2019. With the plan scrapped, some electric vehicle stock investors are wondering what the impact of the decision is. 

According to Ford CEO Jim Farley, part of the breakup was due to the complexities of marrying their hardware and software together. In light of this, the company also reiterated that such a decision has not affected their relationship. Ford continues to believe in the company’s future, retaining a 12{cfdf3f5372635aeb15fd3e2aecc7cb5d7150695e02bd72e0a44f1581164ad809} stake in the EV start-up. The legendary automaker aims to produce 600,000 vehicles per year by the end of 2023. What’s more, Ford has several interesting products including the Mustang Mach-E SUV and the F-150 Lightning pickup truck. If anything, the old dog seems to be learning new tricks quickly. 

The decision to cancel the joint development was mutual, according to Rivian. “As Ford has scaled its own EV strategy and demand for Rivian vehicles has grown, we’ve mutually decided to focus on our own projects and deliveries. Our relationship with Ford is an important part of our journey, and Ford remains an investor and ally on our shared path to an electrified future,” the company said in an email statement.

Zoom (ZM) Reports Earnings After The Closing Bell 

Zoom Video Communications (NASDAQ: ZM) will report its earnings after the stock market closes today. There’s no question that ZM stock has been under pressure heading into that announcement. Investors are less enthusiastic about the company’s short-term growth as more people return to the office. In addition to that, the increased competition from other players in the video conferencing space may unnerve some investors. In light of all this, it shouldn’t come as a surprise that ZM stock remains under considerable selling pressure.
